North Coast Long-Term Services and Support Collaboration

Mission: The North Coast Long-Term Services & Supports Coalition works toward a community in which older people and those with disabilities are given meaningful choices, have access to affordable, coordinated services, a high quality of life and care and support for their family caregivers.

Defining Long-Term Services and Supports: A wide range of personal, medical and social/financial assistance needed by persons with functional limitations over an extended time. Services may be publicly or privately funded, delivered in a wide-range of settings, and may change as the needs of the individual who uses the services change.

The NCLTSSC is made up of over 20 leaders in Humboldt County programs and organizations that provide these long-term services and supports. The focus of the NCLTSSC through June 2012 is to develop a model for Humboldt County in restructuring our long-term services and supports system for adults who are functionally impaired. This model can potentially be replicated in the 27 other counties who, like Humboldt County, are without managed care plans.

The work of the NCLTSSC is supported by a grant a grant from the SCAN Foundation. The SCAN Foundation is dedicated to creating a society in which seniors receive medical treatment and human services that are integrated in the setting most appropriate to their needs.  For more information, please visit www.TheSCANFoundation.org.
